Transitional Housing Program
Screening Form

Sometimes you need a safe place to land while you get back on your feet. Our transitional housing program offers shared-living spaces for up to three years for individuals who are experiencing homelessness or are at risk of losing their housing. While you are with us, we connect you with financial education, personal development resources, and health support. Because stable housing is just the first step.

Please take a moment to answer the questions below. This helps us understand your situation and make sure this program is the right fit for you. If you qualify, you will receive a link to the full application. If this program is not the right fit, we will do our best to connect you with other resources in our community.
